You're one step away from being able to change your name! Fill the deed poll form below with the required details.

Your download will be a zip file containing a PDF image. You'll need a program such as WinZip to open your free deed poll document.

You must be at least 16 years of age to complete this deed poll form.

  1. Fill in your old name, new name, address, and the names and addresses of your witnesses. Your witnesses should not be family members, but people who know you well, . You can pick almost any new name you like, but there are a few restrictions you should read first.
  2. Print your new Deed Poll onto nice, heavy paper (ideally parchment paper). Sign all the bits you need to sign, and have your witnesses sign the bits they need to sign.
  3. Contact all the people who need to know about your new name, a list can be found on who to inform.

Your Details

Once you've entered your new details, scroll down to enter your witness details. When you've completed all the details, click on the download button, found at the bottom of this form.
You'll then be prompted to save your deed poll document.
